The Delta Dental plan allows you to go to any provider you choose. There are two networks of dentists to choose from, Delta Dental PPO network and Delta Dental Premier® network. Your out-of-pocket costs will likely be the lowest if you use Delta Dental PPO provider.

You pay an annual deductible before the plan covers basics and major services. The deductible does not apply to preventative and diagnostic or orthodontic services. The plan pays a percentage of the charges for your treatment, up to the maximum annual benefit and also provides orthodontic benefits for your children with a separate lifetime maximum.
