Who Is Eligible?


All regular employees (hourly and salaried) are eligible to participate in the Lincoln Flexible Benefits Program. You may also enroll your eligible dependents for medical, dental, vision, critical illness, and group accident insurance coverage. Eligible dependents include your spouse, natural, adopted, or step children up to age 26, and unmarried children, regardless of age, who are disabled and considered your dependents under federal income tax rules established by the IRS.

Changing Coverage


You may change your or your dependent(s) coverage during the year only when you experience a qualifying Life event, such as marriage, divorce, birth, death, adoption, placement for adoption, or loss of coverage. The change must be made within 31 days of the event. The change must also be consistent with the event. All changes will be made online. To initiate a change to your benefits, log onto the Fidelity NetBenefits portal. Supporting documentation is to be uploaded.

*Qualified Family Status Change Rules do not apply to the Health Savings Account (HSA). You can begin, end or modify your annual HSA contribution at any time. 

How You Pay for Coverage


Your cost for Medical, Dental and Vision Insurance, as well as Flexible Spending Accounts and Health Savings Account. Contributions are deducted from your pay on a pre-tax basis.

Your cost for Critical Illness, Accident, Supplemental Life Insurance (up to $40,000), Additional Life Insurance and Additional LTD is deducted from your pay on an after-tax basis.

SOCIAL SECURITY NUMBERS REQUIRED FOR ALL COVERED INDIVIDUALS – EMPLOYEES AND DEPENDENTS
Social security numbers are required for all dependents. During open enrollment, employees enrolling in medical plans with dependent coverage must enter their dependents’ social security numbers.
